CIRCULATED TO ALL P&I MEMBERS

Members will be aware that most incidents and accidents that occur on ships can be attributed to ‘human error’.  Recently North’s Loss Prevention Department has been focusing  its attention on the human aspect of incidents and, in particular, on learning lessons from incidents that may be used to reduce future claims exposure for individual Members and the Club as whole.

As part of this focus it has become apparent that incidents cannot be viewed in isolation but rather that they form part of a wider crewing system, and that North’s current data in respect of the crewing of entered vessels is insufficient to place incidents in context. We are therefore requesting background information on crewing from Members in order that we are better able to learn lessons from incidents, and to place them in the wider context of a Member’s entire fleet and our membership as whole. This should assist us in our loss prevention efforts for individual Members and the Club going forward.

It would be greatly appreciated if Members or their crewing providers could complete the data in the attached document and return it to loss.prevention@nepia.com by Friday 19 December 2014.
